When to arrive
Please arrive 15 minutes prior to your scheduled appointment time.
All co-pays are paid on the date of your visit. We accept cash, Health Savings Account(HSA) Flexible Spending Account (FSA)and all other major credit cards.
What to bring to your appointment
You Must bring your current ID, current insurance card(s) and any referrals to your appointment. If you do not you will be billed for the appointment. Please also bring any medication list and any other pertinent information.
